I have a derm who is very helpful. I have Ps and maybe PsA. I would like to try a drug that will be good for both ailments. She has given me the option of requesting what drug I would like to try.

If you had your choice what would you choose?

Big Grin  For me that is obvious, it’s DMF.
But you don’t have that in Canada, so it is no option for you.

Next in my view, seen from sideways and the stories here in the forum, I personally would go for Cosentyx, which is mostly aiming at psa but seems to address both.
But more and more bio’s are being positioned as working for both.

For me it could only be a Bio, they gave me my life back. I was at a point where I could hardly move, I could not dress myself easily or lift a cup to drink from. That all changed after my first bio (Remicade) and since then I have stayed on Bio's, having said that they can stop working but I've never experienced those bad days again and there is always another Bio to try.

But what you have to remember is "We are all different" what works for me may not work for you, I have found that most of the Bio's work for both psoriasis and psoriatic arthritis. Some are better than others, some fail quicker than others, it's all about trying to find the one that gives you the best results till it fails.

If pushed I would say: Stelara, Coesntyx or Tremfya but again that is only my opinion from what I have tried over the years. 

Sounds like you have a good relationship with your dermatologist, my advice would be to have a read through the two threads below and pick something you feel you would like to try. Maybe find 3 options and put them in the Search facility and see what others have posted.

Stelara with out doubt, still working after many years and no problems or significant side effects.

I'm with Caroline and would pick DMF but as has been said it's not available in Canada. It has given me around 9 years clear.
But I have spent many happy hours reading others experiences on here and your best options are bios if your dermatologist will prescribe it and its not too expensive for you I think either Stelara or cosentyx would be my preferred next treatment if ever my drug fails. Fred has given you links to the biological treatments to read which should help
I would try to avoid oral treatments if you can as they might work but may have more side effects than a bio.

I would try Stelara first.  The bonus with this biologic is that the injections are once every three months!

I agree that I would like a biologic. I don't know much but somehow I think they are better than other choices like methotrexate.

Is there one that stands out over the others for the percentage of relief? Some have pretty low rates of efficacy. Like only 50% of the test subjects see only 20% relief. Is 20% worth risking cancer?

If you Google Stelara you will get a lot of information plus there is a lot of information on the site. If you look at the way that it works you will find that it interacts with IL 12 and IL 23 which can be an advantage dealing with P and PsA. I have used MTX and it worked up to a point for me for quite a few years however Stelara was fantastic and you have the first injection then the second at 4 weeks then every 12 weeks. By the time I was ready for the 12 weeks injection I was almost clear. In my opinion there is no question which way to go.
Its 7 years now that I have been on Stelara and it's still working with the only thing that I am had to do is drop to 10 week instead of 12.

You are very unlikely to get cancer form a bio.

Put your choice in the Search facility, tick the box "search titles only" and you will a lot of information, Stelara for example will give you just under 100 threads to look at.
